The Delaware Advance Health Directive Form (Medical Power of Attorney) is a document executed by an individual (referred to by state law as the Declarant) in order to create a point of reference for their health care wishes. The Declarant does so as a preparatory measure in case one day they lack the capacity to personally convey them. In such circumstances, an Agent chosen by the Declarant will be responsible for both communicating such wishes and making important decisions about their end-of-life care on their behalf.
