Oklahoma Minor Child Power of Attorney Form
The Oklahoma Minor Child Power of Attorney is a document that contains provisions regarding a parent or legal custodian’s determination to delegate decision making powers to another party (referred to as the Attorney-in-Fact) so that they can temporarily administer care to their child/ren on an as-needs basis. A parent or legal custodian who wishes to execute one should bear in mind that the laws governing Minor Child Power of Attorney relationships are contained in the Oklahoma Partnership for School Readiness Act and not the Oklahoma Uniform Durable Power of Attorney Act. They should also be mindful that the delegation of power may last for a period of up to one (1) year.

Signing Requirements (§10-701) – In order to be binding and official, the POA must be:
- Signed by the parent or legal custodian;
- Signed by the Attorney-in-fact; and
- Notarized.
