Oklahoma Minor Child Power of Attorney Form
An Oklahoma minor child power of attorney form is a legal document that outlines a parent or legal custodian’s intent to delegate decision-making authority to another individual. This person, known as the attorney-in-fact, provides temporary care of the child or children. The Oklahoma Partnership for School Readiness Act contains regulations governing minor child power of attorney relationships (opposed to the Oklahoma Partnership for School Readiness Act). The delegation of authority can last up to one (1) year.

Signing Requirements (10 O.S. § 701): For the form to be legally binding, a parent or legal custodian attorney-in-fact must sign, and a licensed public notary must endorse it.
