Pennsylvania Limited Power of Attorney Form

A Pennsylvania limited power of attorney is an agreement between two (2) parties that states how and when an agent will perform tasks for a principal. It is unique compared to other types of power of attorneys because it gives restricted power to the agent (instead of broad decision-making abilities). If the agent makes unwanted choices, they are legally and financially responsible for the outcome.

The principal must sign this document twice. A signature under the “Notice” section confirms that they understand the purpose of a limited power of attorney. By signing under the “Signature and Acknowledgment” section, they give an agent permission to act on their behalf.

Signing requirements (§ 5601(b)): Requires two (2) witnesses and notarization.
