Louisiana bill of sale form is utilized by a seller to describe how and when the transfer of an item occurred. The exchange can occur in three ways: 1) cash or check, 2) through trade, or 3) by gift. While not all transactions require a bill of sale form, the purchase of watercraft or a car requires it. This step is mandatory because the Office of Motor Vehicles (OMV) needs a bill of sale to register a boat or vehicle.

What is a Louisiana Bill of Sale?

A Louisiana bill of sale initiates the displacement of goods from one person to another. It tells authorities and third parties that the buyer has taken the title of “owner.”

The person can use the document to show someone they own the item, particularly when handling official transactions. For instance, an insurance company could require the individual to send a signed bill of sale form before providing them with a policy. In addition, the new owner can use the form to re-sell the item in the future if they ever wish to do so.


Registration Forms: Boats

Mail registration forms or submit them in person at a Louisiana Department of Wildlife & Fisheries (LDWF) Office.

  • Bill of Sale – A proof of purchase between a boat owner and purchaser.
  • Boat Registration + Motor Title Application (Form 55) – Used to apply for a boat or motor title registration.
  • Tax Payment Certification for Boat Registration (Form R-1331) – A document that proves the owner has paid sales tax on the boat.
  • Payment – Fees depend on the boat’s length and range between $36 and $99.
  • Certificate of Title – Only considered valid if signed. If the form has been damaged or lost, the owner must fill out Form 55.
  • Driver’s License – The owner must show their official Louisiana driver’s license identification (ID) to prove their identity. If the owner does not have a driver’s license, they must present another form of government ID, such as a passport or birth certificate.

Registration Forms: Firearms

Louisiana does not require gun registration or a permit to buy. Private sellers do not need to conduct background checks on buyers as long as the individual does not have a criminal or mental health history. Firearm holders can apply for a concealed carry license in person, by mail, or online.  

  • Louisiana Concealed Handgun Permit Application (Form DPSSP 4645) – The state provides this document and includes instructions for applicants to complete it. An online version is available for those who wish to apply remotely.
  • Payment – Fees depend on the applicant’s age and the permit’s duration.
  • Proof of Training – The person applying must prove they can responsibly use and possess a firearm by showing a certificate of completing a firearms safety course. For the state to accept the certification, a verified instructor must teach the class.
  • Proof of Residency – A Louisiana driver’s license or state ID card.
  • Fingerprint Card – A trained technician must take the fingerprints in ink. The cards must have the applicant’s credentials, including their name, signature, address, birth date, social security number, and physical identifiers.
  • Additional Information – If applicable, the person applying must disclose a history of divorces,  legal separations, and criminal history or charges. They must also include whether they were in the military or have any current medical conditions.

Registration Forms: Vehicles

Register a vehicle in Louisiana by locating an OMV Office.

  • Bill of Sale – A completed form signed by the buyer, seller, and a notary public.
  • Odometer Disclosure Statement (Form DPSMV 1606) – Verifies the mileage at the time of the sale.
  • Vehicle Application (Form DPSMV 1799) – The vehicle owner completes this document to receive a registration.
  • Payment – Fees depend on the type of vehicle and its weight. The office will also charge state and county taxes upon registration.
  • Insurance Policy (Minimum Requirements) –  
    • $15,000 for bodily injury (per person);
    • $30,000 for bodily injury (2+ people); and
    • $25,000 for property damage.
  • Certificate of Title – The owner must present the original copy. If the original document has been lost or damaged, the owner must complete Form DPSMV 1799.
  • Louisiana Driver’s License – An unexpired government-issued card from Louisiana.

Registration Forms: Trailers

Louisiana trailer registration and titling occur through the OMV.

  • Bill of Sale – To prove their ownership, a buyer uses this form (which requires notarization).
  • Payment – The owner must pay for general registration and titling fees, plus state and local taxes.
  • Identification (ID) – Louisiana requires a photo ID of the applicant to confirm their identity.

Additional (Optional) Documents

  • Homemade Trailer Affidavit (Form DPSSP 4117) – Residents use this document to confirm they have built a homemade trailer and define its purpose (i.e., boat, utility, farming, etc.).
  • Manufacturer’s Statement of Origin (MSO) – Owners of new trailers must present this form to the OMV.
