How to Write
Step 1 – Identifying Information of Owner and Attorney-in-Fact
In this first section, write the current date, the vehicle owner’s first and last name, and the attorney-in-fact’s name and address on the lines provided.

Step 2 – Description of Vehicle
Provide the following information about the vehicle:
- Make (e.g., Honda)
- Model (e.g., Accord)
- Body type (e.g., sedan)
- Year (e.g., 2014)
- VIN (Vehicle Identification Number)

Step 3 – Authorized Transaction Types
This section indicates what the attorney-in-fact is allowed to do on the owner’s behalf.
- Check each box that the representative is authorized to perform.
- If a transaction is not listed, check the ‘Other’ box, and specify it on the line provided.

Step 4 – Vehicle Owner Information
Complete the following about the vehicle owner:
- Check the box to indicate either owner is an Individual or Business
- Signature
- Printed name
- Address
- Phone number
- Email address

Step 5 – Notarization
This form requires the owner to sign in the presence of a notary. Take this form and acceptable document(s) for verification (e.g., driver’s license or passport) to a Notary Public. The Notary Public must enter the county name in which they are located. In addition, they must include the name of the vehicle owner and the county and state where they reside. To complete the form, the Notary Public needs to enter their commission number, the date of signing, and their official signature.
