Arizona Vehicle Power of Attorney (Form 48-1001)
An Arizona motor vehicle power of attorney (Form 48-1001) is a document required by the Arizona Department of Transportation (ADOT) to allow a representative (the attorney-in-fact) to handle vehicle-related matters for a car or truck owner. The attorney-in-fact presents the form to the ADOT to transfer, duplicate, or request a new title. If the owner cannot buy or sell the vehicle, the attorney-in-fact can complete the task on their behalf. The form is only considered genuine if it states the names and addresses of both parties. The owner must include their driver’s license number and date of birth, as well.

Step 1 – Details of Vehicle
Provide the key identifiers of the vehicle in the first section:
- Vehicle Identification Number (VIN);
- Year (e.g., 2010);
- Make (e.g, Volkswagen); and
- Body style (e.g., SUV).
Step 2 – Attorney-in-Fact Information
Write the following about the person appointed as the attorney-in-fact:
- Name;
- Mailing address; and
- City, State, and Zipcode.
Step 3 – Owner Information
In this section, input the following information about the vehicle owner:
- Name;
- Driver’s license number;
- Date of birth;
- Mailing address;
- City, state, zip code; and
- Signature.
Step 4 – Witness
