Arizona Motor Vehicle Power of Attorney (Form 48-1001)
The Arizona Motor Vehicle Power of Attorney (Form 48-1001 rev. 12/13) is a document issued by the Arizona Department of Transportation (ADOT), Motor Vehicle Division (MVD). ADOT is the state agency in charge of managing motor vehicle affairs, including Power of Attorney designations. By completing this form, a motor vehicle owner can nominate an Attorney-In-Fact to sign all of the papers and documents required to process matters pertaining to the title of their motor vehicle.

Signing Requirements – Arizona Department of Transportation requires the motor vehicle owner to sign the form. The Department also required the form to be acknowledged before a Notary Public or a Motor Vehicle Division Agent.
