Iowa Minor Child Durable Power of Attorney Form
Upon its lawful execution, an Iowa Minor Child Durable Power of Attorney will bestow temporary guardianship powers to an individual nominated by a parent or guardian, who must exercise them to ensure the care of their child. This individual, referred to as the Agent, must act in the ways and in the circumstances the parent or guardian sets out in this form. For instance, the Agent may be required to mind the child on specific days of the week, take them to recreational activities, or consent to their medical treatment.

Signing Requirements (§ 633B.105) – Parents or Guardians are legally-bound to sign the the Minor Child Durable Power of Attorney document before a Notary Public or other authorized individual.
