Oregon Minor Child Power of Attorney Form
The Oregon Minor Child Power of Attorney is a form that provides a solution for any parents or guardians who require outside assistance to continually guarantee the care of their child or children. It does so by allowing them to state in a legally-binding manner that should they be unable to take care of their child/ren in the circumstances they have specified, the party named in the form (the Agent) will be authorized to step in for them. In most cases, these circumstances will be related to unforeseen emergency situations, personal commitments, or job responsibilities.

Signing Requirements (§ 109.056) – No signing requirements are specified for a Minor Child Power of Attorney to be put into effect. However, they should consider that it is standard advice of legal professionals for the parents/guardians should sign the Power of Attorney before a Notary Public as well as two (2) disinterested witnesses.
