Utah Minor Child Power of Attorney Form
For a period of up to six (6) months, parents or guardians are legally entitled to delegate the occasional care of their child/ren to another party by executing a Utah Minor Child Power of Attorney. As § 75-5-103 specifies, so long as the powers the parents or guardians choose to delegate do not involve consenting to marriage or adoption of the child/ren, the powers may encompass any matters related to the child/ren’s:
- Care,
- Custody, or
- Property.

Signing Requirements (§ 75-5-103) – State law does not mandate any specific signing requirements for a Minor Power of Attorney. Given the considerable implications of this legal document, it is strongly recommended that parents and guardians sign the document before a Notary Public, and if possible, two (2) witnesses.
