Washington Minor Child Power of Attorney Form
The Washington Minor Child Power of Attorney will in effect allow a parent or guardian to delegate particular parental or guardianship duties to another party known as the Agent. It is necessary for the parents or guardians to clearly note any particular directions or conditions that they want the Agent to follow, or, as RCW 11.94.010 states, the several general provisions noted down will invariably be applied to any Minor Child Power of Attorney. For instance, if the parent or guardian does not want the Agent to make health care decisions for their child/ren on their behalf, they must explicitly state so in the form.

Signing Requirements (RCW 11.125.050) – Attestation by two (2) or more witnesses of the parents’ / guardians’ signatures or acknowledgement of their signatures before an individual who is legally able to accept acknowledgements is demanded by state law.
