Colorado Motor Vehicle Power of Attorney | Form DR 2175


The Colorado Motor Vehicle Power of Attorney (Form DR 2175) is the official form that should be completed when a person (“grantor”) wants to designate someone they trust (their agent) with the power to carry out tasks relating to their motor vehicle. The form was created by the Colorado Division of Motor Vehicles (DMV) to standardize the process of applying for a POA concerning a car, truck, or other vehicle. To use the form, the person completing it will place a checkmark (✓) next to each power they wish their appointee to have. For any options they don’t want the agent to have, said options should be crossed out. Additionally, the grantor is required to write the day, month, and year in which the agreement will terminate.

State Laws: § 15-14-701 and § 15-14-705

Signing Requirements (§ 15-14-705 “Execution of power of attorney”): To be valid, the grantor needs to both print and sign their name, and have the contract be acknowledged by a notary public. The agent (person receiving the powers), does NOT need to sign the form.

Required Motor Vehicle Information: The grantor will need to enter the following information regarding the vehicle they are executing the POA for:

  1. Vehicle Identification Number (VIN)
  2. Year
  3. Make
  4. Model
