Nebraska General (Financial) Power of Attorney Form
Nebraska is one of only a few states that offer a ready-to-use Power of Attorney (POA) Form. Issued by the Nebraska State Unit on Aging, the Nebraska General Power of Attorney (Form MLTC-PB-3) is a comprehensive legal document that covers a range of Power of Attorney situations across four (4) pages. If an individual (referred to as the Principal) wants to include terms that will make their Agent’s authority extinguish upon their incapacitation (therein making it a General POA), they can clearly state so in the “Special Instructions” section of the form.

State Laws – Nebraska Uniform Power of Attorney Act (Sections 30-4001 to 30-4045)
Signing Requirements (§ 30-4005) – Signing requirements for a General Power of Attorney are as follows:
- The Principal (or another individual directed to do so in their conscious presence) must sign or mark the form, and
- A Notary Public or other authorized individual must acknowledge the form.
