New Jersey Tax Power of Attorney (Form M-5008-R)
The New Jersey Tax Power of Attorney (Form M-5008-R) is an official document that allows a taxpayer to designate and subsequently grant a Representative(s) “the authority to obligate, bind, and/or appear” on their behalf before the New Jersey Division of Taxation. Upon filing the form with the Department, any acts the taxpayer has authorized the Representative to conduct will come into effect. The Representative will subsequently be authorized to:
- Receive their confidential tax information, and/or
- Perform any and all acts regarding their taxes that they have detailed in the form.

Signing Requirements – The State of New Jersey Division of Taxation demands that the following parties sign their name on New Jersey Tax Power of Attorney (Form M-5008-R):
- The taxpayer, and
- The Representative.
