Durable Power of Attorney
Designed for long-term financial planning, this form appoints an attorney-in-fact whose authority over the principal’s assets, banking, and investments persists regardless of any future mental disability. The contract remains in force until death or written revocation.
General (Financial) Power of Attorney
Provides the representative with blanket powers over the principal’s financial life, covering bank accounts, assets, and property management. Unlike the durable version, this arrangement is voided if the principal loses mental capacity.
Limited (Special) Power of Attorney (PDF)
Restricts the agent to highly specific tasks such as signing a check, overseeing a property sale, or handling a single business transaction. The form is non-durable and expires once the designated duties are completed.
Medical Power of Attorney
Names a primary agent and optional successor agents to communicate the principal’s healthcare wishes when they are unable to do so themselves. The appointee works directly with physicians to ensure treatment aligns with the principal’s stated preferences.
Minor Child Power of Attorney
Allows a parent or guardian to appoint an agent with temporary caregiving authority over their child. A separate POA document must be completed for each child, and the caregiver may consent to medical care, handle school matters, and tend to daily needs.
Motor Vehicle Power of Attorney (Form MV-008)
Required by the DMV for authorizing another person to sign vehicle-related documents such as registration applications and title transfers on the owner’s behalf.
