What is a Health Template?

Patients and individuals use a health template to monitor wellbeing, create a health or fitness plan, or manage medical forms. If the person uses the formatted document for self-monitoring, it does not need to be signed or notarized. However, if the template has a legal use, such as a DNR or HIPAA form, it must adhere to state signing and notarization laws.
