Oregon Tax Power of Attorney (Form 150-800-005)
Officially named the Tax Information Authorization and Power of Attorney for Representation, the Oregon Tax Power of Attorney (Form 150-800-005) is a form that the Oregon Department of Revenue has issued to allow a taxpayer to:
- Provide authorization to the Department to disclose their confidential tax information to the designee they have named in the form, or
- Authorize a person to represent them before the Department, who will have the power to receive confidential information and make decisions on their behalf.
The taxpayer should note the following considerations as they complete Form 150-800-005:
- The Department only allows for one of the two options noted above to be selected.
- The designee they name in the form may be a person, agency, firm, or organization.

Signing Requirements – The Oregon Department of Revenue requires the taxpayer to sign the form in order for it to be filed. If the taxpayer is also requesting joint representation, the spouse must additionally sign the form.
